Personal Tax Return Preparation Documents Checklist

Tax documents change every year according your circumstances and changes in tax code. The documents that you will need are:

  • W2 Form (Wage and Tax statement) from your employer
  • Rental Income and Expenses 
  • Rental Asset information - Closing document/HUD statement
  • Retirement Income (1099-R) and contributions 
  • Social Security/RRB income (SSA-1099, RRB-1099)
  • Form 5498 showing IRA contributions (If not done by employer through W2)
  • Investment/Portfolio/Savings Income. Interest, dividend income (1099-INT, 1099-OID, 1099-DIV)
  • Income from sales of stock or other property (1099-B, 1099-S)
  • Dates of acquisition and records of your cost or other basis in property you sold (if basis is not reported on 1099-B)
  • Expenses related to your investments
  • 1099-B and/or 1099-DIV from cryptocurrencies, NFTs and all other similar type income or loss. Cost of all cryptocurrencies, NFTs and other similar type of income sales
  • State refunds (Form 1099-G)
  • Unemployment Income 1099-G 
  • Income from K-1s either as investment or as active owner
  • Gambling income (W-2G or records showing income, as well as expense records)
  • Royalties (1099-MISC)
  • Foreign income if applicable 
  • Any other 1099s received
  • Health Savings Account (HSA) Form 1099-SA (distributions from HSA)/5498-SA (contributions)
  • Home Ownership - Form 1098 mortgage interest statements & property tax records
  • Charitable Donations
  • Medical Expenses
  • Health Insurance
    • Form 1095-A if you have health insurance through a State Marketplace (Exchange)
    • Form 1095-C if you have health insurance through your Employer 
  • Childcare Expenses
  • Educational Expenses - Form 1098-T from educational institutions
  • Form 1098-E for student loan interest
  • IRS letters or notices (if any) 
  • Estimated tax payments
  • Tax credits documents including Clean vehicle, Education and Home energy efficiency 


If you are a new client, filing your taxes first time with us, please provide all of the following via our secured portal. 


  • Copy of your previous year filed tax return, showing your, spouse and dependents social security numbers (SSN). If hidden in the tax return, please provide that via our secured portal
  • Dates of birth for you, spouse and dependents 
  • Your unexpired government issued ID (DL preferred)
  • Your spouse's unexpired government issued ID (DL preferred)
  • Identity Protection PIN, if one has been issued to you, your spouse, or your dependent by the IRS
  • Bank routing and account numbers to receive your refund by direct deposit or pay your balance due if you choose
